The Norwegian Petroleum Safety Authority (PSA) has identified a number of breaches of regulations in the March incident investigation on the Åsgard B platform operated by Equinor. At the time, a forklift operator was hit by an 83-pound trolley while dismounting a winch. The technician survived.

The investigation concludes that there was inadequate safety equipment and lack of communication between the employees performing the work. Based on the report, PSA made a series of recommendations to Equinor on operations at the Åsgard B facility, which should be in place by October 1.
